ProWorks Insurance Services (hereafter referred to as “ProWorks”) is provided with information from its customers and former customers (hereafter referred to as “Clients” or “Client”) that is used in evaluating their application and servicing their insurance policy(ies). ProWorks reserves the right to collect non-public, personal information about its Clients from any of the following sources:

I. Forms and applications sent to ProWorks from Client, including but not limited to:


a. ProWorks’ “Proposal Questionnaire” form.
b. Insurance Quotations/Proposals provided to Client by ProWorks.
c. Personal/Business checks, Bank/Cashier’s Checks, Credit Card transactions.
d. Applications and Supplements requested of Client by ProWorks.
e. Any other communiqué sent or otherwise transferred to ProWorks from Client.


II. Reports received from external agencies including Insurance Companies, State Department of Motor Vehicles, Contractor’s State License Board, Credit or Consumer Reporting bureaus, and any other source available to us. Reports from said agencies include:


a. Reports of Claims or “Loss Runs” provided by Insurance Companies.
b. Worker’s Compensation or Client History reports from external Insurance Companies or Agencies
c. Driver’s License Record or Motor Vehicle Report provided by the California Department of Motor Vehicles via a third-party Driving Record provider.
d. Contractor’s License status and other license-related information provided by the California Contractor’s State License Board (CA CSLB), usually via their website (www.cslb.ca.gov).
e. Credit Reports provided by any Credit Reporting Agency or Licensed Bonding Company.
f. Certain Consumer or Credit reports received from Consumer/Credit reporting agencies. Please see “Important Notice”.


Disclosure of Collected Client Information

ProWorks does not disclose any nonpublic information about its customers or former customers to any parties other than those who require the information to provide services to Client, except as mandated by law. In some cases this may mean information can be disclosed to third parties without Client’s authorization. Third parties that may be provided with Client’s nonpublic information include all external agencies from which ProWorks receives information from as listed above.

ProWorks restricts access of nonpublic client information to employees who require such information in order to provide Client with products, benefits or other services. ProWorks maintains physical, electronic, and procedural safeguards that comply with state and federal regulations to guard Client’s nonpublic personal information.

Client has the right to obtain access to certain items of information that have been collected. Client has the further right to request correction of information if it is felt that the information is inaccurate.

Important Notice

IN COMPLIANCE WITH THE REQUIREMENTS OF THE FAIR CREDIT REPORTING ACT (PUBLIC LAW 91-508), PROWORKS INSURANCE SERVICES ADVISES THAT AS PART OF OUR ROUTINE PROCEDURE IN REVIEWING APPLICATIONS FOR INSURANCE OR RENEWALS OF INSURANCE POLICIES, WE MAY PROCURE A CONSUMER REPORT INCLUDING INFORMATION AS TO THE CONSUMER'S CHARACTER, GENERAL REPUTATION, PERSONAL CHARACTERISTICS OR MODE OF LIVING. IF INSURANCE IS FOR AN INDIVIDUAL AND IS PRIMARLY FOR PERSONAL AND FAMILY OR HOUSEHOLD PURPOSES, SUCH INFORMATION MAY BE OBTAINED THROUGH PERSONAL INTERVEIWS WITH NEIGHBORS, FRIENDS OR OTHERS WITH WHOM THE CONSUMER IS ACQUIANTED.

UPON REQUEST TO PROWORKS INSURANCE SERVICES, CLIENT WILL BE PROVIDED, IN WRITING, A COMPLETE AND ACCURATE DISCLOSURE OF THE NATURE AND SCOPE OF THE CONSUMER REPORT REQUESTED OR WILL BE ADVISED THAT NO INVESTIGATION WAS CONDUCTED.
